Copyright/License: some changes how they are included in source

* Making sure that there are docstrings in all module source files, and
using hashes to comment the license notice.
* Adding EXCEPTIONS-CLIENT with the FOSS License Exception, pointing to
the www.mysql.com website.
* Some more updates here and there, but nothing to the code itself.
