Adding support for LCOV-style flags for excluding certain lines from
coverage analysis. This supports the following in-code annotations:
- GCOVR_EXCL_START / LCOV_EXCL_START
- GCOVR_EXCL_STOP / LCOV_EXCL_STOP
- GCOVR_EXCL_LINE / LCOV_EXCL_LINE
