Merged testing improvements (no longer using nose).

This brings 3 improvements:

 * Testing is now done with pure unittest, doctest instead of nose
 * Better CouchDB testing using desktopcouch test idioms (thank you, Chad
   Miller)
 * Tests are run using standard `setup.py test` instead of `make-test` script
