Revise packaging copied from 10.10 for 11.04.
In 11.04: remove packaging for fis-gtm-5.3000, fis-gtm-5.3000A, and fis-gtm-5.4000.
In 11.04: add changelog entries for fis-gtm-5.4000a and openvista-gtm-integration.
In 11.04: fis-gtm-5.4000a: change maintainer to Derek Veit and change required gcc from 4.1 to 4.4.
In 11.04: set GTM ICU version to 4.2 rather than the value provided by icu-config, because GTM-V5.4-000A will not work with ICU 4.4 installed in 11.04 by default.
