Merge set_progress_handler-2468

Change Axiom's QueryCounter utility to explicitly pass a bytecode count
to the PySQLite2 set_progress_handler API, as is required.  The value
passed is 1, which was the default with the unofficial patch exposing
this feature.

Also change some left-over tests which were counting bytecodes directly.
They now use QueryCounter.
