Correctly close and remove a snap-decision notification (visually and from internal queue) upon close-signal, time-out and invoked actions. Fixes LP: #1093114
Make sure an incoming snap-decision (SD) directly replaces any open non-SD. Fixes a regression.
Also disable timed-out signal-callback for snap-decisions, as the telephony-app use-case doesn't work with it correctly. Fixes: https://bugs.launchpad.net/bugs/1093114.
