Add basic support and initial sphinx docs.

This branch adds sphinx docs under the docs directory. Build targets are added
to the Makefile as `doc` and `doc-open`. Just some initial notes are added.
The goal isn't to write out great docs, but to start a place for docs to get
added as required.

The install.txt and the todo.txt have been removed/added to the sphinx docs.
