lookup_tables changes

- Add the get_row method in ci_lookup_table objects. This method add support
  for named columns in lookup tables.

- Add the ci_lookup_table_open, ci_lookup_table_search, ci_lookup_table_release_result,
  and the ci_lookup_table_get_row functions which should be used instead of 
  calling directly the ci_lookup_table methods.
  
- use ci_buffers_* interface to allocate memory for bdb_tables and ldap_tables.
