[r=allenap][bug=][author=allenap] Improve coverage reporting in MAAS.
Switch to coverage 4.1, which seems to work a little better than the
version in Xenial. Always measure branch coverage. Omit tests and
testing resources from HTML coverage report, and include the Bazaar
revision number in the title.
