Ensure creation of Avatar directory before calling expiry logic.

This also deletes some inefficient 'look before you leap' code and
replaces it with a single startup attempt at creating the directory,
which ignores the resulting error when the directory already exists
(this is more efficient because it only checks once at launch rather
than every time an avatar is downloaded).
