* added functionality to installer to rewrite config.php with or without index file depending on whether mod_rewrite is detected
* added 2 columns to s_types: 'is_index' to indicate it is an index content type, and 'parent_type' to indicate what the parent content type is
* added full installer path to form action attributes in installer script
* fixed get_parents_dropdown() to get only appropriate parent types based upon is_index and parent_type variables
