Refactor manual provisioner
- Moved the bulk of the code to a new package, environs/manual.
- Added instance.HostAddresses.
Still to come:
- Set a "provider" field in machine state.
- Update InjectMachine to allow nonce to be specified,
or just use a AddMachineWithParams.
- Handle machine "destruction" (serivce deregistration).
