Address issue found by Alexander Belchenko <bialix@ukr.net>: initial
commit on new branch is recorded as an 'init'.  Change how we record
branch initialization by setting a flag on the branch on post_branch_init
and use it in post_branch_change_tip to decide if the previous
'init' entry (recorded in post_branch_init) should be replaced
because of a 'bzr branch'.  Also enhanced tests.
