* Fix erratic kpartx behaviour:
- Use helpfully undocumented -s (synchronous) option to make behaviour
non-racy.
- Clear loop devices from device-mapper table (failure to do this
occasionally chokes kpartx).
* Unwind correctly on failure to mount 2nd partition.
