Improve healt_lab . Include lower and upper limits for lab tests. Normalize models

Include lower and upper values so now we can compare to the lab test results on each element.

Normalize model names :
gnuhealth_test -> gnuhealth.lab.test
gnuhealth.test_type -> gnuhealth.lab.test_type
