Include text qualitative tests and patient contextual warnings and remarks

- Added 'excluded' flag for each analyte in the model. This way we can exclude specific analyte from the report
- Added 'result_text' field to take into consideration the non-numeric results for specific analytes (morphology, color, ... )
- Add 'remarks' for each analyte.
- Add 'warning' flag. The warning flag will be set automatically when the values of the analyte are out of range. The doctor can also set it when the patient conditions (eg, comorbidities, age, gender...) will require special attention to that value.
- Don't show the sequence in the lab tree view.
- Show the reference (range) values in the tree view, and the upper and lower limit in the form view
- Improve size and fields in the lab odt report
