Various minor changes:
- Update Makefile (don't add vscode files to the .xpi archive)
- Update palemoon version
- Fix delete cookies on Firefox nightly (48.0)
- Don't open the classic preference window, using a modal dialog. It make the Firefox tab system break in some way.
