Fix copyright notices and licensing prior to release.

Compared to the last public release:

 * New files are copyright Canonical Ltd. and licensed under GPLv2+.
 * Files which were not substantially modified use the original license (lesser
   BSD, except for one file copyright Robert Collins which is GPLv2-strict)
 * Files which were substantially modified are lesser BSD with parts copyright
   Canonical Ltd. under GPLv2+.

Work from Canonical is licensed under GPLv2+ and not the original lesser-BSD
license because Canonical contributions significantly surpasses the original
code both in volume and in usefulness. The license of the original code is
GPL-compatible.
