Add support for hwe-generic/ bootloader

Use ubuntu.ins, initrd.off, initrd.siz files from the d-i build,
instead of regenerating them.

Copy hwe-generic/ files into ./boot-hwe cdrom directory.

Generate ./ubuntu-hwe.ins pointing at ./boot-hwe bootloader.

Adjust README.boot to explain ./boot-hwe bootloader.
