Re-enabled PDFBOX-420 (https://issues.apache.org/jira/browse/PDFBOX-420).
In this patch, I removed pemanent mapping from Identity-H to
Adobe-Japan1-UCS2 in the PDFBOX-420. Instead, generate UCS2 mapping name
from DESCENDANT_FONTS as pdfminer[1] does.
